North Merrick Kindergarten registration
Sign-ups are coming.
North Merrick kindergarten registration will be held Tuesday, January 24, and Wednesday, January 25, 9 a.m. - 2 p.m. at Harold D. Fayette Elementary School, 1057 Merrick Avenue in the gym.
The child must be five years old by December 1, 2012.
The requirements needed are the original and one copy of the following documents: child’s birth certificate, passport-size photo of child, proof of residency (owner): tax bill (renter): lease agreement and landlord’s tax bill, driver’s license and yearly physical.
The physical records must include Immunization record:
(3) D.P.T.
(1) Mumps or (3) Polio (1) Rubella
MMR#1(3) Hepatitis B (2) Measles
MMR#2 (1) Varicella
